Driggs and Victor Health Clinics are both a Patient Centered Medical Home. A Patient-Centered Medical Home (PCMH) is a model of primary care that emphasizes coordinated, comprehensive, and continuous care centered on the patient’s needs. It uses a team-based approach led by a primary care provider to manage all aspects of a patient’s health—physical, mental, and social. The goal is to improve health outcomes, enhance patient satisfaction, and reduce costs through better communication and care coordination.
Victor Health Clinic offers a welcoming patient-friendly atmosphere and ample parking. Located on the north side of Victor, it shares a building with Victor Drugstore for easy access to fill prescriptions and medical supplies.We provide family health services for all ages with onsite pediatricians available every Tuesday and Thursday.

As a federally-designated patient-centered medical home (PCMH), you’ll benefit from a carefully-designed team approach to help with your health and wellness goals. PCMH care provides easier accessibility to your care providers, treatments plans, and any referring specialists. Both Driggs and Victor Health Clinics are PCMH-certified.
