Driggs Health Clinic

Driggs and Victor Health Clinics are both a Patient Centered Medical Home. A Patient-Centered Medical Home (PCMH) is a model of primary care that emphasizes coordinated, comprehensive, and continuous care centered on the patient’s needs. It uses a team-based approach led by a primary care provider to manage all aspects of a patient’s health—physical, mental, and social. The goal is to improve health outcomes, enhance patient satisfaction, and reduce costs through better communication and care coordination.

Driggs Health Clinic showcases a perfect blend of tradition and innovation. The clinic is the site of the original Teton Valley Hospital built in 1938 and the historic front facade has been carefully preserved within the reception area. The wide selection of specialty services offered by Driggs Health Clinic represent up-to-date medical care, ranging from behavioral health to sports medicine to chronic care management.

As a federally-designated patient-centered medical home (PCMH), you’ll benefit from a carefully-designed team approach to help with your health and wellness goals. PCMH care provides easier accessibility to your care providers, treatments plans, and any referring specialists. Both Driggs and Victor Health Clinics are PCMH-certified.
