What is Swing Bed Transitional Care?
Swing Bed Transitional Care helps you return to your home when released from the hospital following a procedure or illness. Additional recovery time and rehabilitation are often needed
before transitioning home. Swing Bed Transitional Care at Teton Valley Health provides 24/7 skilled nursing care and essential therapies to prepare patients for a successful return home
following an inpatient hospitalization. With compassionate care and 7-day per week therapy, patients can heal and gain strength to transition back to their own home safely to enjoy their
daily activities. The average length of stay is 1-2 weeks, but each patient has different needs.
Who pays for Swing Bed Transitional Care?

Why is it called Swing Bed?
The Swing Bed concept allows hospitals in rural areas to offer services for both acute care and rehabilitation care. The hospital can use the same facilities for both types of patients, or “swing”
the use of the room or bed between acute care patients and rehabilitation patients.
